Welcome to the Derbyshire Dales Primary Care Network (PCN) — where ten local GP practices work collaboratively to deliver high-quality, accessible care for our communities across the Derbyshire Dales. Together, we believe in putting patients first, supporting well-being, and strengthening local health services for everyone.

We are working together to provide extended services.

What is a Primary Care Network?

A Primary Care Network (PCN) is a group of local GP practices that work together to support patients in their community.

The Derbyshire Dales Primary Care Network (PCN) supports GP practices and patients across the local area, working together to improve health, wellbeing, and access to care for our communities.

Primary Care Networks also work closely with community services, councils, and voluntary organisations to support people with both medical and non-medical needs.
